To cut down the paperwork processing time, please print, fill out, and bring the new patient forms to the office on your first visit.

What are warning signs of gum disease?

1. Red, swollen, or tender gums
2. Bleeding while brushing or flossing
3. Gums that pull away from the teeth
4. Teeth that are loose or are separating from each other
5. Pus between the gums and the teeth
6. Persistent bad breath
7. A change in the way your teeth fit together when you bite
8. A change in the way partial dentures fit.
