Terms of Service
Important: These Terms govern use of the website. They do not replace any treatment consent, financial agreement, office policy, Website Privacy Policy, or Notice of Privacy Practices that applies to dental care or patient information.
1. Acceptance and Scope
These Website Terms of Service (the “Terms”) apply when you access or use https://www.7daydental.com and website features that link to these Terms (collectively, the “Site”). By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.
The Site supports the following separately organized California dental practices, each doing business as 7 Day Dental:
- Christine Chung DDS, Inc.— Laguna Woods office.
- Christine Chung Dental Corp.— Mission Viejo office.
- Chuck Truc Le DDS, Inc.— Lincoln Avenue office in Anaheim.
- Chuck Truc Trong Le Dental Corp.— Euclid Street office in Anaheim.
In these Terms, “7 Day Dental,” “we,” “us,” and “our” mean the practice responsible for the applicable office or Site interaction. The shared name and website do not combine the separate legal entities or make one practice responsible for another practice’s care or records.
2. Website Information Is Not Dental Advice
Site content is provided for general educational and informational purposes. It is not a diagnosis, treatment plan, prescription, or substitute for an examination and advice from a licensed dentist or other qualified health care professional who knows your circumstances. Do not delay or disregard professional care because of information on the Site.
Using the Site, sending a message, requesting an appointment, or calling an office does not by itself create a dentist-patient relationship. A professional relationship is established only when an applicable practice accepts you for care and a licensed provider performs an appropriate evaluation or otherwise agrees to treat you.
3. Emergencies and Time-Sensitive Concerns
The Site and website forms are not continuously monitored and should not be used for emergencies. If you believe you have a life-threatening emergency, call 911 or go to the nearest emergency department. For an urgent dental concern, call the appropriate office directly. If an office is closed or cannot assist promptly, seek care from an appropriate emergency or urgent-care provider.
Website content, office hours, and appointment tools do not guarantee immediate, same-day, after-hours, or emergency treatment.
4. Appointment and Contact Requests
An online appointment or contact submission is a request only. An appointment is not confirmed until the applicable office communicates confirmation. Availability, office hours, providers, services, and scheduling options may vary by location and may change.
You agree to provide accurate and current information and to update the office if relevant information changes. Separate office policies, patient forms, treatment consents, financial arrangements, and appointment communications may apply. If those documents conflict with these Terms on a clinical, scheduling, or financial matter, the more specific document controls for that matter.
5. Services, Insurance, Fees, and Results
Descriptions of services, insurance participation, financing, payment options, fees, promotions, and estimated costs are general information only. Not every service, provider, schedule, insurance arrangement, or payment option is available at every location.
You are responsible for confirming coverage and benefits with your dental benefit plan. Verification of benefits, a preauthorization, or a cost estimate is not a guarantee of payment by an insurer or a final statement of patient responsibility. Treatment recommendations and actual charges depend on an individual evaluation, services provided, and applicable agreements.
Dentistry involves individual factors and no particular clinical, cosmetic, financial, or scheduling result is promised by the Site. Testimonials, ratings, photographs, and descriptions of prior experiences are not guarantees that another person will receive the same result.
6. Electronic Communications
When you submit a form or request, you authorize the applicable practice and its service providers to respond about that request using the contact information you provide. Ordinary email and text messages may not be secure. Do not include highly sensitive medical, financial, or identification information in a general website message. Use a designated patient form, secure channel, or telephone when directed by an office.
These Terms do not provide consent for promotional calls or text messages. Marketing communications will be sent only when separately permitted by law and, when required, separately consented to. Consent to marketing is not a condition of receiving dental care. If you separately agree to receive text messages, message and data rates may apply, and you may request that messages stop using the method stated in the message or by contacting the applicable office.
7. Children and Authorized Representatives
The Site is intended for adults, including parents and legal guardians. Children under 13 should not submit information through the Site without the involvement of a parent or legal guardian. If you submit information or make a request for another person, including a minor, you represent that you are authorized to do so.
8. Privacy and Health Information
Our Website Privacy Policy explains how information collected through the Site is handled. The applicable practice’s Notice of Privacy Practices explains how protected health information is used and disclosed and describes patient rights under HIPAA and other applicable law.
If these Terms conflict with the Website Privacy Policy, a Notice of Privacy Practices, or applicable law concerning personal or protected health information, the privacy document or law controls for that information.
9. Acceptable Use
You may use the Site only for lawful, personal purposes. You may not attempt unauthorized access; interfere with the Site’s operation or security; introduce harmful code; impersonate another person; submit information you are not authorized to provide; or copy or use Site materials for an unauthorized commercial purpose.
We may limit access to the Site when reasonably necessary to address misuse, security concerns, legal requirements, or maintenance. This does not affect a patient’s right to request records or exercise rights through the channels provided by the applicable practice.
10. Intellectual Property
The Site and its original text, graphics, logos, layout, photographs, videos, and other materials are owned by or licensed to the applicable practice or Site operator and are protected by intellectual-property laws. Subject to these Terms, you may view, download, or print reasonable portions for personal, noncommercial use. No other license is granted. 7 Day Dental names and logos may not be used in a way that suggests endorsement or affiliation without written permission.
11. Third-Party Services and Links
The Site may connect to services operated by others, including appointment and patient-form platforms, maps, review services, social media, accessibility tools, payment or financing information, and educational resources. A third party may apply its own terms and privacy practices. We do not control third-party sites and are not responsible for their independent content, availability, or practices, except to the extent responsibility cannot be excluded under applicable law.
A link does not necessarily mean that we endorse every statement, product, or service on the linked site. When a vendor processes information for a practice, the practice remains responsible for its own obligations under applicable privacy and health-information laws.
12. Site Accuracy, Availability, and Changes
We aim to provide useful and current information, but Site content may contain errors, become outdated, or be unavailable. Providers, office hours, services, accepted plans, fees, promotions, and other information may change. Please confirm important information directly with the applicable office before relying on it.
We may update, suspend, or discontinue Site content or features when reasonably necessary. We may also revise these Terms. Revised Terms apply prospectively when posted with a new effective date. Material changes will not retroactively alter rights or obligations that arose before the change unless allowed by law and clearly agreed.
13. Website Disclaimer
We make reasonable efforts to keep the Site useful, secure, and available, but the Site and its general information are provided on an “as is” and “as available” basis to the extent permitted by law. We do not guarantee that the Site will always be available, accurate, error-free, or free of harmful code.
This Section applies only to the Site and its general content. It does not limit any duty relating to professional dental care, patient records, informed consent, privacy law, or another obligation that cannot lawfully be waived.
14. Limitation of Liability
To the extent permitted by law, the applicable practices and their website service providers are not responsible for indirect or consequential losses caused solely by use of, or inability to use, the Site or an independent third-party site.
This limitation does not apply to professional dental services, duties concerning protected health information, gross negligence, willful misconduct, personal injury caused by a legally actionable act or omission, or liability that cannot lawfully be limited.
15. California Law
California law governs these Terms, subject to any rights that applicable law does not permit to be waived.
16. General Terms
If a provision of these Terms is found unenforceable, it will be enforced to the greatest extent permitted and the remaining provisions will remain in effect. A failure to enforce a provision is not a waiver. These Terms are the entire agreement concerning use of the Site, but they do not replace or modify agreements and notices that apply to dental treatment, payment, scheduling, privacy, or patient rights.
